The Cochlear Family Programme
Cochlear Family is part of Cochlear’s lifetime commitment to recipients. Cochlear Family was created to help guide recipients through their journey ahead and provide support when needed. Members can connect with a supportive and inspiring global community of other recipients to hear their stories and learn from their experience.
The Cochlear Family Achieve Award
The Cochlear Family Achieve Award celebrates and honours recipients who have overcome challenges associated with hearing loss and are excelling in non-academic pursuits. From mastering athletic endeavours to artistic vocations, we want to recognise recipients’ determination, resilience, and unwavering spirit in achieving their potential.
Who is eligible to apply?
- Applicants must be a Cochlear™ Nucleus®, Baha®, or Osia® recipient.
- Applicants must be a citizen and resident of the United Kingdom or Ireland.
- Cochlear employees or their family members are not eligible to apply.
- Each winner can only receive the Cochlear Family Achieve Award one time.
When does the Cochlear Family Achieve Award application process start?
- Applications will open in January of each year.
- Completed applications must be submitted via email by March 31st of each year (incomplete applications will not be considered).
- Completed applications must be emailed to ukawards@cochlear.com, posted applications are not accepted.
What is the value of the award?
The winner will be awarded £1,500. The runner-up will be awarded £500. See terms and conditions for more detail.
How is the winner selected?
The winner of the Cochlear Family Achieve Award is chosen based on their story submission of achievement and a demonstration of commitment to the ideals of leadership and humanity. Consideration is also given to the applicant's extracurricular activities, awards or community involvement. Applications go through a points-based review by an internal committee and the final winner and runner-up is selected by a committee of external hearing healthcare professionals.
How will the winner be notified?
We will contact the winner and runner-up via email within three months of the deadline date.
Cochlear Family Achieve Award terms and conditions
Cochlear Europe Limited (“Cochlear”) offers an Achieve Award for recipients of Cochlear™ Nucleus®, Baha® or Osia® systems, and who meet the criteria described below (the “Eligibility Criteria”). All applicants agree to these terms and conditions.
ELIGIBILITY CRITERIA
- Winners and runners-up of the Achieve Award must meet the following Eligibility Criteria:
- Reside within and are citizens of either the United Kingdom or Ireland.
- Are at least 18 years of age, or if under 18, have the written consent of their legal parent or guardian;
- Are not the child of a director, officer, or employee of Cochlear (“children” include natural, adopted, and step children and legal wards);
- Are a Cochlear Nucleus, Baha or Osia recipient.
APPLICATION SUBMISSION
- Submission. Achieve Award applicants must apply by completing the application form on the Cochlear website at www.cochlear.com. Applications must be submitted via email to ukawards@cochlear.com, in the designated file formats. No other means of delivery will be accepted.
- Deadlines. Applications must be submitted by 11.59pm BST on March 31st each year.
BASIS OF SELECTION
The pool of applicants will be evaluated, and the finalists will be selected, based on the following process.
- First Review. The initial pool of applicants will be reviewed by a team of at least 3 Cochlear employees and narrowed down to no more than ten applicants based on the following scoring criteria:
- Story submission (1-5 points)
- Additional Extracurricular/Volunteer/Community Experience (1-5 points)
- Reference (1-5 points)
Those ten applicants with the highest total number of points combined from each Cochlear judge are selected as the finalists. These finalist applications are sent to the Achieve Award Selection Committee.
- Selection Committee. The Selection Committee is a group of independent clinicians who are an Ear, Nose, and Throat Doctor, an Audiologist, an Auditory Verbal Therapist, a Speech Pathologist, or other professional involved in the care of hearing loss. Each judge on the Selection Committee will use a scoring form to evaluate each applicant based on the following scoring criteria:
- Story submission (1-5 points)
- Additional Extracurricular/Volunteer/Community Experience (1-5 points)
- Reference (1-5 points)
The applicant with the top score will be selected as the winner. The applicant with the second highest score will be selected as the runner-up. In the event of a tie the external judges will make a second review of the tied applications and make a final decision based on the criteria stated above. Each year, Cochlear will award one winner and one runner-up.
ACHIEVE AWARD TERMS
- Amount. The amount paid to the winner of the Achieve Award shall be a total of £1,500 (€1,750 if in Ireland). The amount paid to the runner-up shall be a total of £500 (€600 if in Ireland).
- Payment. Payment of the award amount will be made by 30th September of the same year. Cochlear shall pay the award amount in vouchers directly to the winner and runner-up via email. Vouchers can be redeemed at www.love2shoprewards.co.uk.
TIMETABLE OF EVENTS
The Achieve Award shall be administered according to the following timetable:
- Application. All applications must be submitted via email to ukawards@cochlear.com, including any supporting materials, no later than 11:59pm March 31st each year.
- Selection and Notification of Winners. The winner and runner-up will be selected according to the process described above. Winners and non-winners will be notified that they have been selected or not via email to the email address indicated in the application, no later than June 30th of the same year.
- Acceptance of award. No later than 14 days after notification, the winner and runner-up must accept the award in writing by replying to Cochlear at ukawards@cochlear.com. Cochlear reserves the right to select an alternative winner or runner-up should the deadline pass.
ADDITIONAL TERMS
- Media Appearance and Release. The winner and runner-up agree to participate in media activities as requested by Cochlear from time to time in connection with the Achieve Award, including without limitation being featured in Cochlear’s social media channels, and participating in events if requested. The winner and runner-up will be required to sign a media release form provided by Cochlear after written acceptance of the award.
- General. Please see Cochlear’s Terms of Use and Privacy Policy for more information. No purchase or payment is required to apply for the Achieve Award. Cochlear shall not be liable for any lost, delayed, or damaged applications or materials, any faulty transmission or technology, security or other issues on the Cochlear website, or any acts, omissions, or negligence of anyone participating in the judging or selection of Achieve Award recipients. Cochlear reserves the right to cancel, suspend, and/or modify the application process, or any part of it, if any fraud, technical failures, or any other factor impairs the integrity or proper functioning of the process, as determined by Cochlear in its sole discretion, and Cochlear disclaims any liability for such events.
